First Visit Information

We strive to provide same-day treatment whenever possible; however, this cannot be guaranteed on the day of your consultation.

To help us serve you efficiently, please bring the following items to your appointment:

• A current list of all medications you are taking
• Your dental and medical insurance information, including any completed forms (to assist with claims processing)

Important: Patients under the age of 18 must be accompanied by a parent or legal guardian.

Please notify our office in advance if you have any medical conditions that may affect your treatment, such as diabetes, high blood pressure, artificial heart valves or joints, or a history of rheumatic fever. Additionally, let us know if you are taking any medications, including heart medications, aspirin, or blood thinners.

We will send you a new patient form prior to your first visit. Please complete and return it before your appointment.
